To begin working with TherapyCultureLLC, we ask that you take a moment to fill out the Patient Registration form and Informed Consent Agreement located below. Be sure to fill out all the required fields (excluding the bottom highlighted field titled "For Office Use").

Additionally, make sure you submit a picture of the FRONT AND BACK of your Insurance Card to expedite the process.

Once you have done so, send in an email to michael@therapyculturellc.com and click the request appointment button below.
